CMS Releases No Surprises Act Approved Forms

CMS has released documents titled “Requirements related to Surprise Billing: Qualified Payment Amount, Notice and Consent, Disclosure on Patient Protections Against Balance Billing, and State Law Opt-in.” The documents are available through the CMS Paperwork Reduction Act website, here.
